Corewell Health Southfield Center is seeking a Transfer Center professional to facilitate ED-to-ED transfers, relay pertinent patient information, and coordinate transport with Beaumont Health facilities and EMS providers. The ideal candidate will bring EMT licensure in Michigan, BLS certification, and 1 year of relevant EMS or hospital transfer experience to ensure efficient

Qualifications

• Minimum 1 year of relevant experience in EMS or hospital transfer settings.
• EMT licensure in Michigan is required.
• BLS certification (AHA) must be obtained upon hire or ARC equivalent.

Responsibilities

• Facilitate ED to ED transfers between Beaumont Health facilities and standalone EDs.
• Record demographic and clinical information into Transfer Center software and Epic.
• Coordinate with transfer teams and escalate transport needs for air and ground services.
• Relay patient information to receiving facilities and ensure timely communication with transport personnel.
• Enter transport requests and vehicle status data for monthly management reporting.
