Qualifications

• Bachelor's degree in Nursing required and active RN license.
• Minimum 3 years of healthcare experience in acute care or related setting.
• Experience in discharge planning and care coordination is essential.

Responsibilities

• Lead discharge planning and care coordination for ED patients.
• Collaborate with physicians, social workers and case management to remove barriers to discharge.
• Ensure timely documentation and progression notes in the EHR.
• Mentor and support nursing staff and interdepartmental teams.
