Responsibilities

• Lead discharge planning and clinical progression for ED patients
• Coordinate with care team, social work, UM/CDI to ensure timely transitions
• Participate in interdisciplinary rounds and daily progression huddles
• Provide documentation and ensure accurate electronic health record entries
• Mentor and precept newer staff as needed
