Qualifications

• Requires Bachelor’s Degree in Nursing or equivalent.
• Current RN license in Georgia or multi-state compact.

• Strong assessment, interview, organizational and problem-solving skills.

Responsibilities

• Lead and coordinate discharge planning for ED patients.
• Collaborate with ED providers, social work and care coordinators to ensure timely discharge.
• Monitor patient progression toward discharge milestones.
• Maintain accurate documentation in the Electronic Health Record.
• Provide precepting/mentoring for new hires as needed.
