Job summary

Qualifications

• Active RN license (single state) or RN-COMPACT and BLS certification.

Responsibilities

• Lead complex discharge planning and coordinate transitions.
• Assess psychosocial and insurance needs for discharge.
• Document care plans and progress notes in the EHR.
• Mentor staff and participate in precepting new hires.
• Lead interdisciplinary rounds and team huddles to improve throughput.
