Job summary

GBMC Health System seeks a Social Worker in Care Management to assess and intervene for post-discharge care, supervise the ED Discharge Coordinator team, and collaborate with ED psychiatry and nursing. You will interview patients and families, develop care transition plans, and coordinate resources for safe, timely discharges.

The role requires an MSW and 1–3 years hospital discharge planning or crisis experience, with strong assessment and motivational interviewing skills.

Qualifications

• MSW required; licensure in Maryland (LMSW/LCSW) preferred.
• 1–3 years hospital social work or discharge planning experience.
• Crisis and behavioral health experience.
• Knowledge of local crisis/mental health systems.

Responsibilities

• Supervise ED Discharge Coordinator team and coordinate with ED Psychiatry, ED Nursing and providers.
• Perform psychosocial assessments and develop patient care transition plans.
• Arrange timely discharges and coordinate post-acute services with community resources.
• Collaborate with post-acute service liaisons and GBMC/EMS agencies as needed.
• Document assessments and interactions for patient records; participate in ongoing process improvement.
