Northeast Georgia Health System is seeking an experienced ED Admissions Case Manager to evaluate admissions, coordinate appropriate bed placement, and collaborate with physicians and families to ensure a high-quality plan of care and appropriate transitions across the ED and inpatient services.

The role requires GA RN licensure, an associate degree in nursing, and 1–2 years in case management or similar settings.

Qualificações

• RN licensure in Georgia is required.
• Associate degree in nursing or equivalent.
• Experience with utilization management and case management is preferred.

Responsabilidades

• Determine if patients meet national medical necessity criteria for admission/transfer using InterQual/MCG guidelines.
• Coordinate admission to NGMC’s acute beds and communicate with physicians and facilities.
• Refer cases to physician advisors for review and status determination; obtain appropriate status orders.
• Collaborate with physicians, nursing staff and care team to ensure appropriate LOS and timely discharge.
• Communicate with care team to maintain correct patient status and level of care; support customer service goals.
• Adhere to regulatory and payer requirements; understand third-party payers to facilitate outcomes.
• Provide physician education and feedback for status determinations and documentation.
• Maintain patient confidentiality and HIPAA compliance.
• Identify opportunities for process improvements within Care Coordination.
