Job summary

Northeast Georgia Health System is seeking an ED Admissions Case Manager to evaluate and screen ED admissions, ensuring appropriate status and level of care based on physician documentation.

You will collaborate with physicians, patients and families, and the care team to coordinate a comprehensive plan of care, oversee transitions to the next level of care, and provide cross-coverage as needed within the ED Case Management team.

Qualifications

• Licensed RN with GA license; active licensure.
• Associate degree in nursing from an accredited program.
• 1–2 years in Utilization Management using screening guidelines; 3–5 years direct patient care/case management.

Responsibilities

• Determines medical necessity for admission/transfer using InterQual/MCG and physician documentation.
• Coordinates discharge planning and post-acute transitions to ensure timely, safe care.
• Communicates with physicians, care teams, and facilities to confirm appropriate status and level of care.
• Maintains HIPAA compliance and regulatory requirements; protects patient confidentiality.
• Provides cross-coverage for ED RN Case Managers as needed.
