Job summary

PIH Health is seeking an Access Coordinator to support safe, timely patient transitions across the continuum of care. You will work with case managers, social workers, physicians, nurses, and inpatient teams to facilitate throughput, discharge planning, transfers, and repatriation to home facilities.
