Qualifications

• Bachelor’s degree required with related field preference.

• Minimum one year lead or supervisory experience.

Responsibilities

• Provide supervision to assigned program staff and complete required evaluations.
• Track and review daily billing and coding to meet monthly service expectations.
• Coach staff to address performance improvements and maintain quality of care.
• Oversee internal and external program referrals and reporting.
• Assist in developing workflows and support grant writing efforts.
• Participate in staff and department meetings and collaboration with partners.
• Identify community-based partnerships for referrals and outreach strategies.
• Maintain knowledge of Health Plans platforms and documentation.
• Assist in training Care Managers on workflows and templates.
