Job summary

Living Water Clinic-Clinica Agua Viva seeks an ECM Supervisor to lead a team of Enhanced Care Coordinators and Lead Care Managers in Lindsay, CA. This role ensures high-quality, compliant care management aligned with CalAIM ECM requirements and field supervision of staff across multiple service areas.

You will report to the Director of ECM and oversee training, performance, and workflow optimization to improve member outcomes and team effectiveness.

Qualifications

• Bachelor’s degree or equivalent combination of education and experience.
• Minimum 3 years in care management/care coordination.
• Minimum 1 year supervisory or lead experience.
• Experience with Medi-Cal ECM or CalAIM preferred.

Responsibilities

• Directly supervise and develop ECM Coordinators and Lead Care Managers.
• Ensure quality, compliance, and timely outreach across caseloads.
• Monitor productivity, documentation, and performance indicators.
• Coordinate with DHCS requirements and Managed Care Plans.
• Foster a mission-aligned, collaborative team culture.
