Job summary

SAC Health is seeking an ECM Care Manager, RN in San Bernardino, CA to coordinate care for high-risk patients across medical, behavioral, and social domains. The role emphasizes collaboration, mentorship, and using EMR data to guide plans of care.

Schedule is weekdays, 7:30 am–4:00 pm at SBC Clinic with full benefits and potential growth opportunities in a teaching health center setting.

Qualifications

• Associate of Science in Nursing required; BSN preferred.
• Current California RN license and CPR/BLS certification.
• EPIC certification for the module you are hired into.
• 3+ years of acute care, hospital-based experience.

Responsibilities

• Review EMR to provide quality care and develop a plan of care with the provider.
• Communicate with case managers and health professionals to coordinate care.
• Collaborate with physicians, social services, and families on a coordinated plan.
• Promote patient activation toward health goals with evidence-based practice.
• Oversee care coordinators; ensure team cohesiveness and program measures.
• Develop policies and guidelines for patient care and performance.
