Job summary

SAC Health is seeking a Care Manager, Behavioral Health (ECMCMBH) in San Bernardino, CA, to coordinate care for moderate to high-risk patients across medical and behavioral health needs. The role requires MSW, CA driver's license, and ACSW/EPIC experience.

Schedule is Monday–Friday, 7:30 am–4:00 pm with a 30-minute lunch. Join the multidisciplinary team to improve patient outcomes, provide education, and connect patients to community resources.

Qualifications

• Master's Degree in Social Work (MSW) or related field.
• ACSW licensure and CPR/BLS certification preferred.
• CA Driver's License and auto insurance required.
• Experience with case management in community settings is highly preferred.

Responsibilities

• Provide intensive case management for a caseload with complex behavioral and medical needs.
• Conduct initial and ongoing behavioral health assessments and develop care plans.
• Collaborate with interdisciplinary care teams to coordinate services.
• Engage patients and families to promote adherence and education on health goals.
• Conduct home visits as needed and maintain documentation in EHR.
• Meet productivity expectations and present case reviews with the team.
