Qualifications

Responsibilities

• Conduct patient assessments and develop individualized care plans.
• Coordinate and monitor service delivery.
• Maintain detailed progress notes and case records.
• Conduct quality assurance reviews and collect required program data.
• Perform field visits to establish ongoing eligibility.
• Monitor patient safety and ensure services identified in the Individual Service Plan are implemented.
• Address patient concerns and advocate for needed resources.

Access TLC is seeking a compassionate and motivated ECM Care Coordinator to join our San Fernando team. As an ECM Care Coordinator, you will provide primarily in-office case management services while conducting regular field visits to patients in the community. You will play a key role in coordinating care, connecting patients to community-based services, and ensuring timely and efficient delivery of support. Access TLC supports patients by arranging community-based and supportive services that enhance quality of life and promote independence. We are looking for someone who is eager to learn, compassionate, dependable, and a true team player.

Compensation

• $20.00/hour – Without a Bachelor’s degree
• $22.00/hour – With a Bachelor’s degree
