Job summary

Qualifications

• Bachelor’s or Master’s degree preferred but not required for $20 rate.
• Clean California Driver’s License required.
• Reliable vehicle with insurance meeting state and/or organizational requirements.

Responsibilities

• Conduct patient assessments and develop individualized care plans.
• Coordinate and monitor service delivery.
• Maintain detailed progress notes and case records.
• Conduct quality assurance reviews and collect required program data.
• Perform field visits to establish ongoing eligibility.
• Monitor patient safety and ensure services identified in the Individual Service Plan are implemented.
• Address patient concerns and advocate for needed resources.
