Our mission is to provided our clients with a continuum of treatment in order to create opportunities for community reintegration and prevent further hospitalization while maintaining high professional standards.Enhanced Care Management is a whole-person, interdisciplinary approach to care that addresses the clinical and non-clinical needs of high cost and/or high-need Medi-Cal recipients through services and comprehensive care that is community-based, interdisciplinary and person-centered.The Lead Care Coordinator, Enhanced Care Management (ECM) is responsible for evaluating and assessing the needs of clients, identifying the services needed, creating a care plan as well as monthly ongoing care management. This person is a trained and licensed social worker who possesses good communication and interpersonal skills along with strong organizational skills.The Lead Care Coordinator reports to the Practice Manager, ECM.
- Collaborates with ECM team members, enrollees and community partners
- Conducts phone intake assessments, enrollments and ongoing care management plan appointments
- Counsels clients and/or their families to facilitate achieving plan goals, developing life skills, and/or sustaining financial stability, conducts activities and monitors the service environment in a manner to maximize client success and well-being
- Develops and sustains positive relationships with a range of people, including enrollees, community leaders, local groups and organizations, and local authorities
- Supporting face-to-face care management, and online activities with ECM enrollees
- Attends meetings and working as part of the ECM project team
- Maintains case notes and participant files and ensure that all reporting requirements are adhered to as outlined by Sanctuary Centers and ECM program for every client interaction
- Works collaboratively with community partners and other Sanctuary Centers staff in linking clients with needed services
- Advocates on behalf of clients and families for services, basic needs, and other related issues
- Prepares correspondence, case notes, narrative and technical reports, notifications, and related documents using computer-based applications
- Conducts comprehensive, client-centered social work activities in accordance with best practices
- Performs other duties/special projects as assigned

- Knowledge of general interviewing practices and/or techniques; knowledge of social work and counseling practices and techniques, including crisis prevention and intervention
- Work collaboratively to implement the seven core services for ECM:

- Comprehensive transitional care
- Member and family supports
- Coordination of and referral to community and social support services
- Experience of working with a variety of different stakeholders, including residents, local groups, and organizations across the voluntary and public sectors
- Understand, maintain, and uphold the mission and vision of Sanctuary Centers of Santa Barbara
Education And Experience
- Master’s Degree required
- LMFT, LPCC or LCSW or registered AMFT, APCC, ACSW required
- At least two (2) years of experience in social case work, counseling, eligibility determination, social service or community resource referral, or closely related work
- Extensive experience working in community outreach and engagement, particularly with disadvantaged groups
- Strong written and verbal communication skills required
- Strong computer skills are required especially with case management data systems
- Must be hard working and reliable professional willing to be a role model
- Bi-Lingual, English and Spanish
- Must possess a valid driver’s license and motor vehicle insurance to operate a motor vehicle in the State of California
- Must pass a criminal background check
Job Type: Full-time
Pay: $70,000 – $80,000 per year, DOE
