Opportunity Highlights

Enhanced Care Management is a statewide Medi-Cal benefit available to individuals with complex needs. The Care Coordinator will lead the assessment, care plan development and ongoing care management of eligible participants under the supportive guidance of an ECM Supervisor. Our program specifically targets foster youth and other at-risk youth populations, but may also include their family members. The Care Coordinator utilizes motivational interviewing to engage individuals to support them in better managing their chronic conditions. The Care Coordinator actively engages individuals to remove barriers to health, social services, and behavioral health care.

The Care Coordinator Opportunity Includes
  • A job with a purpose; help support youth and families in need
  • Hourly pay rate: $22.00 per hour to start, or higher with prior ECM experience
  • Benefits include paid time off (PTO), paid holidays, and participation in our 401k plan
