Imperial Calcasieu Human Services Authority is seeking a Community Service Professional to serve as the Single Point of Entry Intake Coordinator for the EarlySteps program in the Developmental Disabilities Division. The position focuses on intake processing, referrals, and coordinating with health and community partners to support infants and toddlers with developmental needs.

Qualificações

• Five years of experience in providing health or developmentally disabled services, counseling, health care management, or social services; OR
• Six years of full-time work experience plus two years of experience in providing health or developmentally disabled services, counseling, health care management, or social services; OR
• A bachelor's degree plus two years of experience in providing health or developmentally disabled services, counseling, health care management, or social services; OR
• An advanced degree in a social sciences field, nursing, public health; public, hospital, or health administration; speech-language pathology or communication sciences and disorders; physical, occupational, recreational, or music therapy plus one year of experience in providing health or developmentally disabled services, counseling, health care management, or social services.

Responsabilidades

• Receives, reviews, and processes referrals for individuals seeking access to EarlySteps and other designated OIDD programs.
• Serves as an initial point of contact for individuals, families, healthcare providers, community organizations, and other referral sources.
• Obtains and documents demographic information, referral details, presenting concerns, and other information necessary to initiate the intake process.
• Reviews referrals for completeness and obtains additional information as needed.
• Conducts preliminary intake interviews and gathers information to identify potential service needs and available supports.
• Interviews individuals and families to obtain information regarding developmental concerns, functional needs, existing supports, and available resources.
• Completes designated intake questionnaires and screening activities in accordance with program requirements.
• Coordinates with regional and statewide partners to support effective EarlySteps program implementation, referral processes, and community awareness.
• Represents regional EarlySteps intake operations in meetings with the EarlySteps state office and other program partners.
• Participates in Regional Interagency Coordinating Council RICC), State Interagency Coordinating Council (SICC), statewide EarlySteps meetings, and other designated program activities.
