Position Title: Early Start Intake Service Coordinator

Title of Immediate Supervisor: Early Start Intake Program Manager

Salary Range: $53,666-$78,486

Summary Of Position

This position provides intake assessments for children ages 0-3.

ESSENTIAL DUTIES AND RESPONSIBILITIES

1. Complete intake developmental evaluations for infants and toddlers under 3 years of age who are referred to the Early Start Intake Unit.
2. Work directly with families to schedule in-person, intake evaluation appointments within the natural environment, such as the family’s home.
3. Complete a DAYC-2 developmental evaluation tool, hearing screening tool, and vision screening tool.
4. Complete an M-CHAT-R screening tool when appropriate.
5. Review all case records, including medical and legal documents.
6. Recommend additional evaluations, such as OT, PT, and ST evaluations, when appropriate.
7. Write a psychosocial report including caregiver concerns regarding the infant or toddler’s development.
8. Complete and submit all documentation following agency procedures and timelines.

Qualifications

Minimum qualifications include a Bachelor’s degree in Social Work, Psychology, Human Services, Special Education, or a related field, plus two years of professional experience working with infants and toddlers under 3 years of age. Bilingual English/Spanish skills required. Must possess a valid California Driver’s License.
