San Andreas Regional Center is seeking an Early Start Service Coordinator to coordinate services for infants and toddlers with developmental delays. You will work closely with families, service providers, and multidisciplinary teams to plan, implement, and monitor IFSPs, ensuring children receive needed interventions.

The role requires travel within catchment areas and regular home visits, with a focus on family-centered, natural-environment planning and advocacy to support children's

Qualifikationen

• Bachelor's Degree in Social Work or related field required.
• Knowledge of social work principles and community resources; ability to relate to diverse families.
• Experience with case management and IFSP/IEP processes preferred.
• Ability to prepare and present oral and written reports; maintain case records.

• Develops IFSP in collaboration with families and other professionals.
• Facilitate communication among service providers, therapists, and specialists.
• Coordinate services for infants and families; determine ongoing eligibility up to age 3.
• Complete required forms, documentation, and regulatory reports.
• Promote advocacy by parents and help them become their children's teachers in daily skills.
• Monitor and evaluate services provided through the IFSP and track progress.
