Qualifications

• CT licensure as a Speech-Language Pathologist (SLP) with CCC required.
• Experience serving infants, toddlers and their families.
• Service Coordination training or willingness to obtain.

Responsibilities

• Home visitation with infants and toddlers and their families.
• Conduct developmental screenings and eligibility evaluations.
• Collaborate with clinical team, families and other professionals.
• Develop and implement IFSP plans with families.
• Assist with transitions to preschool and community supports.
• Maintain regular and reliable attendance as essential function.
