Autauga Western Elmore Arc is seeking a Residential Full Time Service Coordinator in Prattville, AL to assist infants and toddlers with disabilities and their families under Part C of IDEA. You will coordinate services, assess development, and develop IFSPs with actionable goals, collaborating with families to implement interventions and monitor progress.

The role requires a Bachelor's degree, valid driver’s license, and strong organizational and communication skills.

Formación

• Bachelor's degree from an accredited school.
• Valid driver’s license and reliable transportation.
• Clear Background check.
• Resume and minimum of three references.
• Strong attention to detail and time management.

Responsabilidades

• Serve as the single point of contact coordinating services under Part C.
• Conduct comprehensive assessments of children's development.
• Gather medical history and family background.
• Create IFSPs with goals, strategies, and services.
• Collaborate with families to set meaningful goals.
• Implement the IFSP through interventions, such as therapy or support services.
• Coordinate provision of early intervention services.
• Monitor progress and adjust IFSP as needed.
• Participate in meetings and evaluations to ensure appropriate services.
• Facilitate transition planning to preschool or other services.
• Document observations, interventions, and progress.
• Document all activities related to service coordination.
