Job summary

Qualifications

• Bachelor's degree in Social Work, Psychology, Education, or related Health and Human Service field required.
• Must complete Introductory Service Coordination Training to be hired.
• Strong organizational and case management skills.
• Excellent communication and interpersonal abilities.
• Ability to work with diverse families from various cultural and linguistic backgrounds.
• Proficiency with computer systems and documentation.
• Bilingual Spanish or Bengali highly preferred.
• American Sign Language a plus.
• Recent college graduates are welcome to apply.

Responsibilities

• Serve as the primary point of contact for assigned families throughout the EI process.
• Coordinate and facilitate IFSP meetings, ensuring family priorities are addressed.
• Monitor service delivery to ensure all authorized services begin within required timelines.
• Educate families on their rights, available services, and the EI process.
• Connect families with appropriate evaluators and service providers.
• Maintain regular contact with families to address questions and concerns.
• Collaborate with providers, EIOD, and other team members.
• Complete all required documentation in NYEIS/EI-Hub accurately and timely.
• Support families through transition planning to preschool services.
• Advocate for families and help them navigate the service system.
