Job summary

The Alliance for Infants and Toddlers is seeking a Service Coordinator to provide coordinated early intervention services to families in Washington County, PA. You will work with families, medical providers, and community resources to ensure timely screenings, assessments, and IFSP planning.

Responsibilities include explaining services, coordinating plans, maintaining records, and meeting all regulatory requirements.

Qualifications

• Bachelor’s degree in early intervention or related field with 1 year of related experience.
• Or an Associate’s degree with 3 years of related experience.
• Knowledge of laws and regulations governing the Early Intervention Program in PA.

Responsibilities

• Explain Early Intervention Services to families and coordinate screenings.
• Identify and coordinate community resources and services beneficial to children and family.
• Coordinate with medical and health providers.
• Monitor child and family needs through use of appropriate instruments and family interviews.
• Provide parent education and support.
• Develop and review the Service Coordination Plan and the IFSP.
• Monitor delivery of Early Intervention Services through regular contact with family and providers.
• Explain parent’s rights and procedural safeguards.
• Initiate development of a transition plan to pre-school services.
• Attend team and staff meetings and required trainings.
• Maintain productivity and complete documentation within agency timelines.
