Job description

Chester County Department of MH/IDD seeks an Early Intervention Service Coordinator to facilitate, coordinate and monitor services for children aged birth to three years old at-risk/with at least a 25% delay in one area of development. Service Coordinators work with families in their homes and/or in the community. This position requires travel to attend meetings off-site.


Service Coordinators are responsible for assisting infants, toddlers and their families in accessing needed Early Intervention services, as well as coordinating and monitoring the provision of such services.


This position is subject to the provisions of a collective bargaining unit agreement.


The position provides case management services to children aged birth to 3 years at-risk/with developmental delays. This includes initial screenings, intake interviews, appropriate referrals, plan development and completion of progress notes and consumer contacts. The specific required tasks of each caseload vary based on the business needs of the department as well as the needs of the consumers. Some positions will have the primary duty of conducting intake meetings and evaluations, while other positions will primarily support children with active IFSPs, although all positions will be expected to complete all duties as a Service Coordinator when necessary.


A person in this position is expected to demonstrate the system of care practice model, which maximizes all resources around an individual or family. The system of care includes child and adult serving agencies and community partners that join with an individual or family to meet their goals.


Duties include:

  1. Assess client needs and develop personal service plans on an annual basis
  2. Assure implementation and coordination of the plan
  3. Oversee the development and implementation of the Individualized Family Service Plan per Act 212 And EI Regulations
  4. Monitor client programs as per established early intervention requirements
  5. Maintain records, correspondence, service plans and authorizations for each client
  6. Make necessary referrals and coordinate services as outlined in the Service Coordination Support Plan (including other medical and community supports)
  7. Have weekly supervisor conferences on specific cases on caseload
  8. Maintain established SC performance expectations
  9. Prepare monthly case management statistics
  10. Attend staff conferences of other agencies as they relate to early intervention supports and services
  11. Assure close working relationship with other human services agencies as they relate to assigned caseload
  12. Initiate and assist with transition to the Intermediate Unit
  13. Close cases no longer necessitating early intervention services
  14. Assist with covering cases when position vacancies exist in the unit.
  15. Other duties as assigned by Supervisor

The Early Intervention Service Coordinator position comes under the provision of the Child Protective Services Law. If you are a final candidate for this position, you will have to provide reports on your background from such sources as the Pennsylvania State Police, Federal Criminal History, and the PA Child Abuse History Clearance. If your background is unacceptable, you will be disqualified for employment in this position. You will receive information about these requirements and how to obtain the required reports at the time you are being considered for employment.


  • Candidate must have a bachelor's degree from an accredited college or university which includes 12 college credits in early intervention, early childhood special education, early childhood education, child development, special education, family counseling, family studies, social welfare, psychology, or other comparable social sciences and 1 year of full-time or full time equivalent experience working with or providing counseling to children, families or individuals with disabilities.

Preferred Skills:

  • Bachelor's degree in Social Science or related field.
  • Two years of experience in the human services field.
  • Experience with individuals who have developmental disabilities.
  • Experience and knowledge in early childhood development.
  • Background and knowledge in social work and psychology.
  • Minimum of one year of experience in the social work field.
  • Proficiency in team facilitation and mediation.
  • Knowledge of Part C of IDEA and early intervention regulation.
  • Knowledge of MH/MR Act 66.
  • Working knowledge of SS/DPW system.

Relevant background checks, clearances, physicals, drug tests, and any other assessments or evaluations will be conducted after an offer of employment is given per County policy.
