Job summary

RWJBarnabas Health seeks a Service Coordinator CSH to coordinate Early Intervention EI services for developmentally delayed children and their families in Bayonne. You will assist with IFSP development and provide counseling and crisis support while advocating for access to needed services.

Qualifications

• Bachelor's degree or higher in a related health, human service, psychology, or education field.
• At least one year of documented professional experience with children and families in counseling, supporting, or case management.
• Experience with birth to five years and their families preferred.

Responsibilities

• Facilitate coordination of EI services for developmentally delayed children and families.
• Develop and review IFSP for Part C children and provide guidance to families.
• Assist with transition planning to preschool or other services as age three approaches.
