  • Two years of experience in a health or human services field.
  • 24 semester hours of education in behavioral sciences, social sciences, or a related field.
  • A combination of education and experience equal to two years in a related field.

Responsibilities

  • Meet with families to review needs, goals, and available supports
  • Help families access services and community resources
  • Coordinate referrals and communication with service providers
  • Assist with Individualized Family Service Plans (IFSPs)
  • Schedule and lead IFSP team meetings
  • Support families in advocating for their child’s needs
  • Identify service gaps and assist with problem-solving
  • Maintain accurate records and follow DHHS guidance, policies, and regulations
