Job summary

Montgomery County PA is seeking an Early Intervention Service Coordinator to manage a caseload and help eligible families locate, coordinate, and monitor mandated services for children birth to 3 years old.

The role includes screening referrals, tracking at-risk children, coordinating evaluations within 45 days, developing IFSPs, and ensuring timely access to services in the child’s natural environment. On-site attendance is required.

Qualifications

• Initial screening of referrals to identify family resources and needs.
• Track at-risk children; coordinate eligibility and tracking services as needed.
• Coordinate Multidisciplinary Evaluation within 45 days of referral.
• Facilitate development, implementation, and review of the IFSP and transition plan.
• Document monitoring activities and maintain case comments; productivity goal 60%.
• Assist families in accessing EI services and community resources in natural environments.
• Attend at least 24 hours of annual training related to EI, child development, or services for children with disabilities.
• Other duties as delegated; on-site attendance required.

Responsibilities

• Coordinate IFSP development and annual reviews and track progress.
• Monitor health and safety concerns identified during visits.
• Arrange access to mandated services in the family’s environment.
• Travel to family homes and community programs as needed.
