Job summary

County Human Services seeks a Human Services Case Manager to provide intake, evaluation, referral, and monitoring for children birth to three in the Early Intervention Program. You will coordinate services, document in the Pelican system, and advocate for families to ensure timely supports.

Role requires a bachelor’s degree in social welfare, state/federal clearances, and willingness to travel with occasional evening meetings. Strong communication and confidentiality are essential.

Qualifications

• Bachelor's degree in social welfare required.
• Clearances: PA State Police, FBI, PA Child Abuse, OIG; maintain clearance.
• Six months of Human Services Case Manager experience or acceptable alternatives.
• Knowledge of early intervention regulations and related state/federal rules.

Responsibilities

• Conduct intakes with children and families; schedule evaluations for eligibility.
• Develop and coordinate Individualized Family Services Plans.
• Monitor services and maintain records in Pelican system.
• Interact with families, agencies and providers; advocate for children and parents.
