Job summary

The Early Diversion Program Coordinator at Community Counseling Center Inc will supervise Outreach Specialists, coordinate grant implementation, and foster collaboration among behavioral health, healthcare, justice, and community partners.

This position oversees day-to-day operations, supports the Early Diversion Coalition, ensures grant compliance, monitors outcomes, and supports community education and training initiatives.

Qualifications

• Masters' Degree in a human services related field
• Bachelor's Degree in a human services related field with at least three years of behavioral health experience post-degree

Responsibilities

• Provide direct supervision, coaching, and performance management for Early Diversion Outreach Specialists, including hiring, onboarding, scheduling, training, and annual performance evaluations.
• Oversee the implementation of the Early Diversion Program to ensure services are delivered in accordance with SAMHSA grant requirements, agency policies, and evidence-based practices.
• Coordinate and facilitate the Early Diversion Coalition, including planning meetings, maintaining partner engagement, documenting activities, and supporting collaborative decision-making.
• Lead the Early Diversion Community System Mapping process utilizing the Sequential Intercept Model (SIM) framework and assist coalition members in developing and implementing the regional Early Diversion Plan.
• Develop and maintain collaborative relationships with behavioral health providers, law enforcement, EMS, fire departments, hospitals, courts, schools, primary care providers, housing agencies, and other community partners to strengthen referral pathways and improve access to services.
• Oversee outreach, engagement, screening, referral, care coordination, and follow-up services provided by Outreach Specialists to ensure timely, high-quality service delivery.
• Monitor program outcomes, performance measures, and quality improvement activities; identify trends and implement strategies to improve program effectiveness and client outcomes.
• Ensure accurate and timely documentation, data collection, and reporting to meet agency, state, and federal grant requirements.
• Prepare reports, summaries, and other documentation required by agency leadership and funding agencies.
• Coordinate the development and delivery of community education and training related to behavioral health, co-occurring disorders, trauma-informed care, crisis intervention, suicide prevention, and early diversion strategies.
• Provide consultation and technical assistance to community partners regarding behavioral health resources, referral processes, and early diversion best practices.
• Work collaboratively with agency leadership to develop policies, procedures, workflows, and sustainability strategies that support long-term success of the Early Diversion Program.
• Participate in grant management activities, including program planning, implementation, monitoring, evaluation, and preparation for site visits or audits
• Assist in identifying community resource gaps and opportunities to expand services through strategic partnerships and community collaboration.
• Document in the electronic health record and other systems as required by programming.
• Ensure compliance with all applicable federal, state accreditation, and agency regulations, policies, confidentiality standards, and ethical guidelines.
• Participate in agency leadership meetings, supervision, professional development, and other activities that support organizational and program goals
• Perform other duties as assigned to support the mission of Community Counseling Center and the objectives of the Early Diversion Program.
