Qualifications

• Master's degree in a human services related field, or Bachelor's in a human services field with at least three years of behavioral health experience post-degree.
• MO Class E Driver's License or ability to obtain by start date, and proof of continuous Auto Insurance.

• Oversee program implementation to meet SAMHSA grant requirements and agency policies.

• Develop and maintain relationships with behavioral health providers, law enforcement, EMS, hospitals, and community partners to strengthen referrals.
• Monitor program outcomes, data collection, reporting, and quality improvement activities.

Schedule: Monday - Friday 8:00am - 4:30pm (Times may vary; occasional evening or weekend maybe required)

Location: Cape Girardeau, MO, must be willing to travel in are catchment areas of Cape Girardeau, Fredericktown, Marble Hill, Perryville, and Ste Genevieve.

Pay Exempt: $47,000 - $52,000 Per Year (Depending on experience)

Qualifications:

• Masters' Degree in a human services related field, or; Bachelor's Degree in a human services related field with at least three years of behavioral health experience post-degree.
• Must hold or obtain by start date, MO Class E Driver's License (Class D for IL Residents), and proof of continuous Auto Insurance.

Program Summary:

Job Summary: The Early Diversion Program Coordinator provides leadership and oversight for the Early Diversion Program by supervising Early Diversion Outreach Specialists, coordinating grant implementation, and fostering collaboration among behavioral health, healthcare, justice, and community partners. This position is responsible for overseeing day-to-day program operations, facilitating the Early Diversion Coalition and Community System Mapping process, ensuring compliance with grant requirements, monitoring program outcomes, and supporting the development of community education and training initiatives. The Program Coordinator works to strengthen early identification, referral pathways, and access to behavioral health and recovery services for adults with mental illness and co-occurring substance use disorders who are at risk of involvement with the criminal justice system.

Job Duties:

1. Provide direct supervision, coaching, and performance management for Early Diversion Outreach Specialists, including hiring, onboarding, scheduling, training, and annual performance evaluations.
2. Oversee the implementation of the Early Diversion Program to ensure services are delivered in accordance with SAMHSA grant requirements, agency policies, and evidence-based practices.
3. Coordinate and facilitate the Early Diversion Coalition, including planning meetings, maintaining partner engagement, documenting activities, and supporting collaborative decision-making.
4. Lead the Early Diversion Community System Mapping process utilizing the Sequential Intercept Model (SIM) framework and assist coalition members in developing and implementing the regional Early Diversion Plan.
5. Develop and maintain collaborative relationships with behavioral health providers, law enforcement, EMS, fire departments, hospitals, courts, schools, primary care providers, housing agencies, and other community partners to strengthen referral pathways and improve access to services.
6. Oversee outreach, engagement, screening, referral, care coordination, and follow-up services provided by Outreach Specialists to ensure timely, high-quality service delivery.
7. Monitor program outcomes, performance measures, and quality improvement activities; identify trends and implement strategies to improve program effectiveness and client outcomes.
8. Ensure accurate and timely documentation, data collection, and reporting to meet agency, state, and federal grant requirements.
9. Prepare reports, summaries, and other documentation required by agency leadership and funding agencies.
10. Coordinate the development and delivery of community education and training related to behavioral health, co-occurring disorders, trauma-informed care, crisis intervention, suicide prevention, and early diversion strategies.
11. Provide consultation and technical assistance to community partners regarding behavioral health resources, referral processes, and early diversion best practices.
12. Work collaboratively with agency leadership to develop policies, procedures, workflows, and sustainability strategies that support long-term success of the Early Diversion Program.
13. Participate in grant management activities, including program planning, implementation, monitoring, evaluation, and preparation for site visits or audits
14. Assist in identifying community resource gaps and opportunities to expand services through strategic partnerships and community collaboration.
15. Document in the electronic health record and other systems as required by programming.
16. Ensure compliance with all applicable federal, state, accreditation, and agency regulations, policies, confidentiality standards, and ethical guidelines.
17. Participate in agency leadership meetings, supervision, professional development, and other activities that support organizational and program goals
18. Perform other duties as assigned to support the mission of Community Counseling Center and the objectives of the Early Diversion Program.
