Job summary

Community Counseling Center is seeking an Early Diversion Outreach Specialist in Perryville, MO. The role focuses on outreach, engagement and care coordination for adults with mental illness and co-occurring substance use disorders, aiming to connect them to timely behavioral health supports before crises escalate.

Responsibilities include community rapport, collaboration with health providers and law enforcement, screenings, motivational support, and linking to treatment and resources.

Qualifications

• Bachelor's degree or equivalent education/experience as listed.
• Four-year combination of higher education and qualifying experience acceptable.
• Four years of qualifying experience accepted for eligibility.
• Must hold or obtain MO Class E Driver's License by start date.

Responsibilities

• Conduct community outreach to identify adults with mental illness and co-occurring disorders at risk of justice involvement.
• Build collaborative relationships with law enforcement, EMS, hospitals and community partners.
• Screen individuals for behavioral health needs and assist early identification.
• Provide brief engagement, motivational support and crisis stabilization services.
• Coordinate care linking individuals to behavioral health and social supports.
• Maintain follow-up and navigation services until long-term linkage to care.
• Educate communities and partners about available behavioral health services and recovery resources.
• Develop and deliver community education on behavioral health and crisis intervention.
• Participate in coalition meetings and coordination of early diversion initiatives.
• Respond with law enforcement and EMS during behavioral health-related calls when appropriate.
• Document outreach activities, referrals and progress in required systems.
