Job summary

Community Counseling Center is seeking an Early Diversion Outreach Specialist in Perryville, MO to connect adults with mental health and substance use supports. You will engage communities, coordinate with law enforcement and healthcare partners, and help individuals access timely treatment and recovery resources.

Responsibilities include outreach, screenings, brief engagement, and care coordination with a focus on preventing crises.

Qualifications

• Bachelor's degree or four-year combination of higher education and qualifying experience.
• Four years of qualifying experience
• Must hold MO Class E Driver's License by start date and proof of continuous Auto Insurance.

Responsibilities

• Conduct community outreach and engagement to identify adults with mental illness and co-occurring substance use disorders at risk of criminal justice involvement.
• Build collaborative relationships with law enforcement, EMS, hospitals, behavioral health providers, courts, and community partners to promote early diversion.
• Screen individuals for behavioral health needs and assist with early identification of mental health and substance use concerns.
• Provide brief engagement and crisis stabilization services to encourage participation in treatment and recovery.
• Coordinate care by connecting individuals with behavioral health, substance use treatment, healthcare, housing, employment, and other supports.
• Maintain follow-up and navigation services until linked to long-term supports (typically up to three months).
• Educate individuals, families, and partners about available services and recovery resources.
• Develop and deliver community education and training on behavioral health, suicide prevention, trauma-informed care, and crisis intervention.
• Participate in coalition meetings and planning to strengthen local early diversion initiatives.
• Respond with law enforcement and EMS during behavioral health-related calls when appropriate.
• Document outreach activities, referrals, and client progress in EMRs and grant-reporting systems.
• Collect and report program data to support grant compliance and evaluation.
• Participate in supervision, meetings, and ongoing training.
• Perform other duties to support the Early Diversion Program and CCC.
