Job summary

Valley View Post Acute is seeking a Rehab Aide-PRN to assist the Physical Therapist in delivering patient care. You will record treatment notes, support the treatment plan, and help prepare the therapy area and equipment for daily sessions.

You will train nursing staff and families in maintenance and discharge preparation, communicate patient progress to the health team, and participate in staff training. A state-licensed Staff Physical Therapy Assistant credential is required.

Qualifications

• Licensed as a Staff Physical Therapy Assistant in the state.
• Proficient in data entry in Casamba and Point Click Care.
• Able to read and interpret policy and procedure manuals.
• Good communication with managers, coworkers and patients’ families.

Responsibilities

• Treat patients as directed by Physical Therapist and record daily treatment notes.
• Assist in maintaining the department and treatment area.
• Instruct patients’ families or nursing staff in maintenance and caregiver training in preparation for discharge from therapy services.
• Report any problems with department equipment and ensure it is maintained in good working order.
• Communicate with supervisor and health team about patient progress and plans.
