Qualifications

Responsibilities

• Transfers patients with assistance as needed, using safe body mechanics.
• Demonstrates safe and appropriate body mechanics to prevent injuries.
• Treats patients with dignity and empathy, staying engaged with those in need of services.
• Designs and updates care plans that integrate goals, outcomes and discharge plans.
• Adjusts care based on patient response and available evidence.
• Educates patients, families, caregivers, students, and other providers using effective teaching methods.
• Evaluates clinical findings to determine PT diagnoses and outcomes of care.
• Performs examinations using reliable methods tied to chief complaint and history.
• Documents per CTSCP67 and regulatory standards.
• Screens patients to determine need for therapy services; uses support personnel appropriately.
• Monitors care delivered by support staff and provides feedback.
• Completes annual education per HIP and REM program requirements where applicable.
• Implements intervention plans considering functional and emotional needs.
