Responsibilities

• Demonstrates ability to perform patient transfers with assistance as needed from bed, chair or floor.
• Uses safe body mechanics to protect staff and patients from injury.
• Designs a plan of care with goals, treatment, outcomes, and discharge plan.
• Educates patients and families using effective teaching methods.
• Evaluates clinical findings to determine PT diagnoses and outcomes of care.
• Performs examinations and documents results per policy.
• Communicates progress and recommendations with referring providers.
