Job summary

Civita Care Center at Cheshire is seeking a dedicated Physical Therapist (PT) to join our team in Cheshire, CT. This full-time position focuses on evaluating residents and designing personalized treatment plans to improve mobility, strength, balance, and overall function.

The role involves collaboration with an interdisciplinary team across post-acute and long-term care, with a strong emphasis on patient education, progress tracking, and evidence-based practice.

Qualifications

• Active Physical Therapist license required.
• Strong communication and clinical reasoning skills.
• Skilled nursing experience preferred but not required.

Responsibilities

• Perform physical therapy evaluations and establish individualized treatment plans.
• Provide treatment focused on mobility, gait training, transfers, balance, strengthening, endurance, and pain management.
• Collaborate with PTA staff, nursing professionals, physicians, and interdisciplinary team members.
• Monitor resident progress and adjust treatment plans as appropriate.
• Educate residents and families on exercises, mobility techniques, and safety recommendations.
• Maintain accurate and timely documentation.
