Job summary

Civita Care Center at Cheshire is seeking a Physical Therapist (PT) on a PRN basis in Cheshire, CT. You will join a collaborative rehabilitation setting focused on helping residents regain mobility, build strength, and increase functional independence.

The position requires an Active CT PT license, a graduate from an accredited PT program, and strong clinical skills. Prior skilled nursing or post-acute rehab experience is preferred but not required.

Qualifications

• Active Connecticut Physical Therapist license required.
• Graduate of an accredited Physical Therapy program.
• Strong clinical, communication, and organizational skills.
• Skilled nursing or post-acute rehabilitation experience preferred, but not required.

Responsibilities

• Complete physical therapy evaluations and develop individualized plans of care.
• Provide skilled interventions addressing strength, balance, gait, transfers, endurance, and mobility.
• Establish appropriate rehabilitation goals and track resident progress.
• Adjust treatment plans based on clinical findings and response to therapy.
• Educate residents and caregivers on therapeutic exercises and safe mobility strategies.
• Maintain clear and timely clinical documentation.
• Coordinate care with PTAs, nursing, and other members of the interdisciplinary team.
