Job summary
Civita Care Center at Milford in Milford, CT is seeking a Physical Therapist Assistant (PTA) to join the rehabilitation team in a skilled nursing and post-acute care setting. This full-time role offers hands-on therapy to help residents improve mobility, build strength, and regain independence; you will work under licensed PT supervision and collaborate with PT, OT, SLP, nursing.

Qualifications
- Active Connecticut PTA license required.

Responsibilities
- Provide physical therapy treatments under the direction of a licensed Physical Therapist.
- Assist residents with gait training, strengthening, balance, transfers, endurance, and functional mobility.
- Carry out individualized treatment plans based on resident needs and rehabilitation goals.
- Monitor resident response to therapy and communicate progress to the supervising PT.
- Reinforce therapeutic exercises, safe movement techniques, and mobility strategies.
- Complete accurate and timely clinical documentation.
- Collaborate with PT, OT, SLP, nursing, and other members of the interdisciplinary care team.
