Job summary

Qualifications

• BLS for Healthcare Providers (AHA) required within 6 months unless exempted by leadership.
• Physical Therapist license issued by the state (Wisconsin or Illinois) required.
• Bachelor's degree in Physical Therapy required.
• No prior experience required.

Responsibilities

• Interpret physician referrals and perform musculoskeletal, neuromuscular, cardiopulmonary, and/or integumentary evaluations.
• Establish a plan of care and implement individualized treatment based on evaluation results and goals.
• Monitor outcomes and adjust treatment plans; communicate updates to physicians.
• Plan and complete patient discharge with home programs and family teaching.
• Document progress, billing, scheduling, and adherence to department policies.
• Maintain clean, organized treatment areas and assist with equipment as needed.
• Utilize modalities and exercises safely; stay current with indications and contraindications.
• Participate in performance improvement activities and support coverage needs.
• Pursue ongoing professional growth and orient new staff as requested.
• Provide age-appropriate care across the lifespan as described in department standards.
