Qualifications

• Basic Life Support (BLS) certification
• Physical Therapist license issued by the state of Wisconsin
• Intermediate computer skills
• Excellent communication and interpersonal skills
• Demonstrated organization, decision‑making, and problem solving skills
• Team members will maintain annual educational requirements for hospital based certifications

Responsibilities

• Interprets physician referrals and completes musculoskeletal, neuromuscular, cardiopulmonary, and/or integumentary evaluations.
• Establishes a plan of care and implements individual treatment based on evaluation results and functional goals.
• Monitors and evaluates outcomes and adjusts the treatment plan; provides status updates to physicians.
• Plans for and completes patient discharge including home programs and patient/family teaching.
• Completes required documentation recording patient progress, billing, and scheduling per policy.
• Responsible for cleaning up after treatment and keeping area neat and orderly.
• Demonstrates use of modalities and equipment, and knowledge of indications/contraindications.
• Participates in performance improvement activities and manages caseload with leadership input.
• Pursues professional growth through continuing education and staff orientation of new members.
• Must provide age-appropriate care and understand growth and development principles; interpret data to meet patient age-specific needs.
