Job summary

Qualifications

• Doctorate degree in Physical Therapy from an accredited school.
• Licensed Physical Therapist in the state of work; BLS certification as required.

Responsibilities

• Interpret referrals and evaluate musculoskeletal, neuromuscular, cardiopulmonary, and integumentary systems.
• Establish a plan of care and implement treatment based on evaluation and goals; monitor outcomes and adjust as needed.
• Discharge planning including home programs, patient/family teaching, and coordination for community services; document and bill per policy.
• Ensure timely documentation, co-signatures, and adherence to department policies and accreditation standards.
