Job summary

Aurora Health Care is seeking a Physical Therapist to evaluate patients and develop individualized plans of care. You will monitor progress, adjust treatments, and coordinate discharge planning with home programs and teaching.

Responsibilities include documentation, scheduling, and ensuring quality care while maintaining a clean, organized work area. A Wisconsin license and BLS are required within six months.

Qualifications

• Bachelor's Degree in Physical Therapy.
• Physical Therapist license issued by the state.
• Basic Life Support (BLS) certification within 6 months.
• No prior experience required.
• Intermediate computer skills.
• Excellent communication and interpersonal skills.
• Demonstrated organization, decision-making, and problem solving skills.
• Annual educational requirements for hospital-based certifications.

Responsibilities

• Interpret referrals and perform musculoskeletal, neuromuscular, cardiopulmonary, and integumentary evaluations.
• Develop and implement a plan of care based on evaluation and functional goals.
• Monitor outcomes and adjust treatment; report to physicians as needed.
• Plan discharge with home programs, teaching, referrals, and equipment.
• Document progress notes, reports, discharge summaries; manage billing and scheduling per policy.
• Maintain clean, organized treatment area and assist with patient handling and positioning.
• Apply modalities and exercise equipment safely; know indications/contraindications.
• Participate in performance improvement activities and manage caseload.
• Pursue professional growth; orient new staff; provide feedback.
• Provide age-appropriate care and assess growth/development per policies.
