Job summary

Aurora Health Care in Wisconsin seeks a dedicated Physical Therapist to evaluate musculoskeletal, neuromuscular, and cardio-pulmonary function, establish care plans, and deliver individualized treatment aimed at functional goals.

You will monitor outcomes, discharge patients with home programs, document care per policy, and participate in continuing education and staff orientation. The role requires BLS within 6 months and Wisconsin PT licensure.

Qualifications

• Bachelor's Degree in Physical Therapy.
• No experience required.
• BLS certification within 6 months and Wisconsin Physical Therapy license required.

Responsibilities

• Interpret referrals and complete musculoskeletal, neuromuscular, cardiopulmonary, and/or integumentary evaluations.
• Establish a plan of care and implement treatment based on evaluation results and functional goals.
• Monitor outcomes and adjust the treatment plan based on patient response and provide status updates to physicians.
• Plan for and complete patient discharge including home programs and family teaching.
• Complete documentation of care, billing, scheduling, and progress notes per department policy.
• Maintain a neat, safe treatment area and operate equipment properly.
• Engage in professional growth and orient new staff as requested.
