Job summary

Aurora Health Care in Wisconsin seeks a licensed Physical Therapist to interpret physician referrals and perform comprehensive musculoskeletal, neuromuscular, cardiopulmonary, and integumentary evaluations to establish a plan of care. Qualifications

• Doctorate degree in Physical Therapy from an accredited school.
• Licensed Physical Therapist in the state where you work; BLS for Healthcare Providers.
• No experience required

Responsibilities

• Interpret physician referrals and complete musculoskeletal, neuromuscular, cardiopulmonary, and/or integumentary evaluations.
• Establish a plan of care; implement individualized treatment based on evaluation results and functional goals.
• Monitor outcomes; adjust treatment plan based on patient response; provide status updates to physicians and teammates.
• Complete documentation per department policy; include evaluations, progress notes, and discharge summaries.
• Coordinate with stakeholders to achieve optimal patient care; ensure safety and infection control measures.
