Job summary

Qualifications

• Graduate of an accredited physical therapy program.
• Licensed in the state by the Board of Physical Therapy.

Responsibilities

• Evaluate new patients and design an appropriate plan of care with measurable goals.
• Perform accurate and timely evaluations within 48 business hours of referral.
• Document services provided and update patient progress regularly.
• Supervise Physical Therapy Assistants and Rehab Techs as needed.
• Coordinate with physicians and other staff for discharge planning.
• Maintain patient dignity, provide patient education, and ensure safety.
