Job summary

Life Care Centers of America is seeking a Physical Therapist to provide direct rehabilitation care under physician referral in accordance with all applicable laws and Life Care standards. The role requires a graduate in physical therapy with current state licensure and CPR certification.

Experience is preferred, but new grads are welcome. Responsibilities include following treatment plans, supervising PTAs and Rehab Aides, documenting care, and collaborating with an interdisciplinary team to

Qualifications

Graduate of an accredited physical therapy program; MSPT or DPT if 2003 or later.
Currently registered/licensed in the applicable state and maintain active licensure.
One year experience preferred; new grads welcome to apply.
CPR certification required upon hire or during orientation.

Responsibilities

Participates in community health matters/projects as appropriate.
Liaisons with patients, families, support departments, etc., to plan for patient needs.
Follow physical therapy treatment plans for patients; establish and modify goals.
Oversee and evaluate care given by PTAs and Rehab Aides; chart appropriately and timely.
Communicate and function on an interdisciplinary team; maintain professionalism.
