Job summary

Life Care Centers of America is seeking a Physical Therapist to deliver direct rehab care to patients upon physician referral, in compliance with all laws and Life Care standards. The role requires an accredited PT degree and active state licensure, with preferred 1 year of experience. CPR must be current.

Responsibilities include planning patient needs with families and departments, documenting progress, and contributing to interdisciplinary teams while upholding professional conduct.

Qualifications

• Graduate of an accredited program in physical therapy (BSPT/MSPT/DPT) or equivalent.
• Currently registered/licensed in the applicable state and maintain active license.
• One year of experience preferred; new grads are welcome to apply.

Responsibilities

• Participates in community health matters/projects as appropriate.
• Liaisons with patients, families, and support departments to plan for patient needs.
• Establish, assess, and modify functional goals for patients.
• Provide direct rehab care following physician referrals and Life Care standards.
• Maintain patient rights, safety, and sanitation procedures.
