Qualifications

• Licensed physical therapist with professional competency as a general practitioner in physical therapy.
• Strong communication with patients and families from diverse ages/backgrounds.
• Demonstrated use of varied evaluation and treatment approaches.

Responsibilities

• Provides evaluation services including review of history, specific assessments, and documentation of results.
• Explains evaluation findings and treatment plan to patient and family; involve patient/family in goals.
• Implements treatment protocols based on evaluation and patient status; adjust plan as needed.
• Completes discharge planning and follow-up care with timely documentation and coding.
• Manage an accurate schedule and uphold productivity while showing empathy.
