Qualifications

• Bachelor's Degree in Physical Therapy required.
• Licensed Physical Therapist (IL IDFPR) required.
• Basic Life Support (BLS) within 30 days required.
• Attend and satisfactorily complete continuing education on acute stroke care.

Responsibilities

• Designs an appropriate plan of care with goals, treatment, and discharge plan.
• Educates patients, families, caregivers, students and other health care providers.
• Evaluates clinical findings to determine PT diagnosis and outcomes of care.
• Performs examinations with appropriate methods based on history and screening.
• Produces documentation per CTSCP67 policy.
• Screens patients to determine therapy needs and effectiveness.
• Supervises support personnel and provides feedback.
• Completes annual education for DNV Hip and Knee program when applicable.
